Please complete the Annual Recertification Packet and return it to the housing authority office by mail or hand delivery. DO NOT Fax the paperwork.

Failure to return ALL information and required documents prior to the deadline on your ANNUAL RECERTICATION NOTICE, will result in the termination of your Housing Choice Voucher subsidy.
The Housing authority is not responsible for lost or undelivered mail. If you have any questions, please contact the housing authority at the number on your ANNUAL RECERTICATION NOTICE. The Housing Authority is not responsible if we are unable to reach you at the contact information that you provided. Please supply valid and current contact information on your application.
After the first missed appointment, you will be mailed a 2nd AND FINAL NOTICE
